Talk to our Student Recruitment Division staff

  • Show us your academic results and ensure that you meet all the entry requirements of the programme.
  • Receive a copy of the programme leaflet/brochure and fee structure.
  • Understand the structure, options and duration of the programme.  (Request for subject exemptions, if applicable.)
Fill up the application form

Use a black or blue ballpoint pen to complete this form. Remember to write clearly in CAPITAL letters in the relevant space given. Complete all sections in this application form.

Submission of Documents

We advise international students to read the admission requirements carefully and ensure all the information submitted is complete.


Student application checklist

  1. One copy of the student’s passport (all pages). The recommended minimum passport validity is 18 months.
  2. A copy of the student’s academic transcripts, completion letter and relevant certificates.
  3. One passport-sized photograph with a WHITE background (35mm x 45mm). (Guidelines »)
  4. Health Declaration Form. Students from Africa, Central America and South America or who have been in transit for more than 12 hours in an airport in a country where yellow fever may occur are required to provide a Yellow Fever Card.

    Health Criteria for Entry of International Students

  5. An English translation of all documents that are not in English, and a Declaration Letter issued by the student’s home country’s Ministry of Education or respective embassy in Malaysia; if the student’s name or date of birth is not the same as that on their passport and/or academic transcript/certificate.
  6. Proof of English Language Proficiency e.g. MUET, IELTS or TOEFL

  8. No Objection Certificate (NOC) only for students from Sudan, Oman and Iran (QIU will apply the NOC for Sudanese, Omanis and Iranian students from their respective embassies in Malaysia)
  9. Libyan students are required to obtain a Letter of Eligibility (LOE) from the Academic Affairs of the Embassy of Libyan in Kuala Lumpur via https://loep-ly.com
  10. China students are required to provide a Verification Report for Academic Certificate. The online verification report can be obtained through :

How can a student make an application?

You may obtain an Application Form from the Student Recruitment Division at the University or apply online here.

When is the closing date for applications?

Applications take place throughout the year for the Foundation Programmes with three intakes in April, July and September/October. Students are encouraged to apply as soon as they have their actual results. However, for the Degree Programmes there are only three intakes per year i.e. April/ May, June/July and September/ October.

When and how do I pay tuition fees?

How do I apply for a credit transfer?

You can apply for a credit transfer after your admission. To do so, you can write to the Student Admissions and Records Division and your Faculty/Centre. The details of the process will be covered by a Faculty Briefing during your Orientation week.

What are the supporting documents that I need before I submit my application?

Malaysian Applicant

  • Scanned copy of Identity Card (NRIC)
  • Scanned copies of all Academic Qualification certificates e.g. SPM / O Level / STPM / A Level / CIMP / SAM / Others
  • RM100 Application Fee (Non-refundable) for Foundation, Diploma and Degree Programmes. RM200 for Bachelor of Pharmacy & RM300 for MBBS

International Applicant

  • Passport Photo against a white background (high resolution) – For more information, please refer to the Passport Photo Guidelines.
    Scanned Passport (all pages including empty page/cover. Validity of the passport must be at least 24 months)
    Certified True Copy of all Academic Qualification certificates e.g. SPM / O Level / STPM / A Level / CIMP / SAM / Others
  • Health Declaration Form.
    • Students from Africa, Central America and South America or students who have been in transit for more than 12 hours in an airport in a country where yellow fever may occur are required to provide a Yellow Fever Card.
  • No Objection Certificate (NOC) for students from Sudan only. This document will be applied by our International Students Office from Sudanese Embassy in Kuala Lumpur.
  • Letter of Eligibility (LOE) for students from Iran. This document will be applied by our International Students Office from Iranian Embassy in Kuala Lumpur.
  • No Objection Letter (NOL) for students from Oman. This document will be applied by our International Students Office from Cultural Attache Office of Embassy of the Sultanate of Oman in Kuala Lumpur
  • Letter of Attestation Declaration from the respective embassy in Malaysia or Ministry of Education in the home country in support of any discrepancy in name or date of birth on the passport and academic transcript/certificate.
  • Application Fee (non-refundable) of RM2300.00 & RM2500.00 for MBBS via Telegraphic Transfer (TT):

Who can certify the above documents?

The above documents can be certified by the School Principal, Division A Government Officers or a Commissioner of Oaths.

Please note that for certain programmes, such as pharmacy and medical programmes, our commitment to admission is governed by criteria set by the Professional Boards, Malaysian Medical Council, the Ministry of Higher Education, Malaysia Qualification Agency as well as the immigration authorities.
